Role of a Case Manager
The case manager’s role is very diverse and ranges from ‘anything to nothing!’
Whilst following the wishes of the client, their family and support network it is the role of the case manager to identify individual need, negotiate referrals to appropriate agencies and encourage participation, monitor, review, support any therapeutic/medical treatments, that may be necessary and facilitate adjustment to life with a permanent brain injury.
The roles and responsibilities of the Case Manager will include the following guiding principles in order to guarantee that the client and their family have the necessary support to live within their community.
- To provide a detailed assessment to establish goals.
- To establish and co-ordinate a care package.
- To act as an advocate on behalf of the client and their family.
- To work closely with the client’s family.
- To network with fellow professionals including other care agencies to maintain an equilibrium in the community for the client and their family.
- To recruit, train and supervise support workers.
- To provide a 24 hour service for out of hours crisis and emergencies.
- To research resources available in the local community and negotiate placement for the client.
- To assist the client and their family in household maintenance, finances and employment of support workers.
Role of a Support Worker/Buddy
The roles and responsibilities of the Support Worker/Buddy will include the following guiding principles in order to guarantee that the client and their family have the necessary support to live within their community.
- To work towards established goals within the care plan.
- To act as an advocate on behalf of the client and their family.
- To work closely with the client’s family.
- To network with fellow professionals including other care agencies to maintain an equilibrium in the community for the client and their family.
- To report to the Case Manager
- To research resources available in the local community
- To assist the client and their family in household maintenance, finances and employment of support workers.
